POLST (Provider Orders for Life Sustaining Treatment) is a form which translates a Health Care Directive into a Medical Order which can be followed by healthcare professionals including EMTs (Emergency Medical Technicians).  Generally used for people at the very end of their lives, the POLST must be obtained through direct contact and discussion with your health care provider (Doctor or Advance Practice Nurse, or in some cases, Physician Assistant).  It is not something you can complete on your own, as it requires the provider's signature to be valid.
